ASU Current and Former Student Name Change
Use this form if you are a currently enrolled, newly admitted/readmitted, or former ASU student and need to update your primary/legal name on your student record.

If you are not currently enrolled, newly admitted/readmitted, or a former ASU student, please refer to the Name Changes for Non-ASU Students section below for guidance.
International Students
If you hold F‑1 or J‑1 visa status, name changes must be processed through the International Students and Scholars Center (ISSC).
If applicable, any international documents supporting a legal name change must be submitted in the original language and accompanied by an official English translation. Translations must be literal, complete versions of the originals and completed by a university or government official, or a certified translation service. Self-translations will not be accepted.

Name Changes for Non-ASU Students
- Faculty and Staff – Please initiate your name change request through Workday or contact your department or unit's HR representative for guidance.
- Air University, ASU Preparatory Academy (K-12) Schools – Please reach out directly to your respective school for assistance with name changes.
- Parents with My ASU Guest Access – See https://tuition.asu.edu/forms/update-parent-plus-borrower-info-request
- Courtesy Affiliates – Individuals with just a courtesy affiliate ID should contact the ASU Experience Center for support or submit a Service Now ticket.
Name Changes and Records from Closed Institutions
If the University maintains records for an institution you attended that has since closed, please review the information below.
The University maintains the official academic records for certain institutions that have closed. In accordance with applicable accreditation and record-retention requirements, these records are preserved in their original form to ensure the accuracy, integrity and authenticity of the academic history at the time of attendance. Because the records are maintained exactly as received from the closed institution, the University does not have the capability to alter, amend, or modify them in any way, including the student name as it appeared at the time of enrollment.
